Cardiovascular disease (CVD) is growing rapidly in the United States, with nearly half the population projected to have some form of CVD by 2030 and associated costs rising from $318 billion to $918 billion during that time. The AHA has a goal of reducing CVD 20 percent by 2020, but it recognizes patients must be consistently engaged and motivated to follow healthy behaviors.
That's why the AHA has teamed up with PatientBond to jointly create and deliver segmented digital care pathways designed to amplify patient engagement using world-class, science based content. Patients opt in to participate and then complete short assessments that allow the platform to tailor educational content according to their health status and personal motivations via psychographic segmentation.

Health Enhancement Program: Manage general population wellness and prevention of cardiovascular disease (90-day program; can be extended based on client need)
Condition Management Program: Manage specific forms of cardiovascular disease as a supplement to providers’ care (12-month program; currently includes Heart Failure, Coronary Artery Disease (CAD), Atrial Fibrillation (Afib) and Stroke. Version 2.0 will add Cardiac Rehabilitation, Hypertension, Coronary Artery Bypass Graft (CABG), Acute Myocardial Infarction (AMI), Peripheral Artery Disease (PAD) and Diabetes.
Readmissions Reduction Program: Reduce hospital readmissions for various cardiovascular conditions (30- to 90-day program)
