c2b Sister Company PatientBond Partners With Cordata to Improve Specialty Care Coordination

Physician consulting with healthcare consumers

Cordata Healthcare Innovations announced a partnership with PatientBond, LLC, whereby PatientBond’s targeted, personalized communications capabilities will be embedded into Cordata’s specialty care coordination platform.

Cordata Healthcare Innovations

Cordata’s specialty care coordination platform helps healthcare organizations more effectively treat people with chronic specialty diseases and generate better clinical and business outcomes through effective patient management and retention along complex care paths. Physicians, nurse navigators, service-line directors and business executives at 100+ hospitals and health systems rely on Cordata for timely and actionable insights.  Cordata features address many critical care coordination needs, including:

  • Oncology: risk scores, patient distress surveys, medication adherence, appointment adherence and follow-up
  • Spine/Orthopedic: patient-reported outcomes (complete joint replacement and Oswestry, e.g.), appointment adherence and follow-up
  • Behavioral Health: patient distress surveys, medication adherence, appointment adherence and follow-up

PatientBond

PatientBond offers next generation healthcare consumer engagement solutions driven by patient behavior change technology.  PatientBond’s flexible communications platform automates emails, texts and phone calls, based on patients’ communication preferences determined through extensive market research and psychographic segmentation.  PatientBond addresses specific engagement needs, including hospital readmission avoidance, appointment and patient payment reminders, medication adherence, disease management and more.

PatientBond utilizes c2b solutions’ psychographic segmentation model and insights to customize messaging according to the recipient’s (healthcare consumer, caregiver, healthcare professional) attitudes and motivations regarding healthcare.  This approach heightens the likelihood of desired behavior change, by appealing to the recipient’s “health personality.”

PatientBond acquired one of the c2b companies – c2b Horizons – in April 2015, giving it access to much of c2b solutions’ proprietary healthcare consumer insights and capabilities.

Partnering to Improve Care Coordination

“This partnership benefits our hospital and health system clients in multiple ways, from focusing care coordination resources on the interactions that matter most, to helping patients overcome barriers to care and stick to their treatment plans,” said Gary Winzenread, CEO of Cordata. “This capability is especially useful for care coordination programs that are often understaffed and overwhelmed. Beyond making coordinators more productive, it also helps hospitals develop and sustain strong and personalized relationships with people needing care for complex or chronic conditions.”

Casey Alberston, President and Chief Operating Officer of PatientBond added, “Patient engagement is a worthy goal, but meaningful behavior change must be the target to improve patient compliance with treatment plans, improve outcomes and reduce outmigration.” Albertson continued by stating, “Partnering with Cordata will allow us to change the behavior of exponentially more people. Thanks to Cordata’s focus on critical hospital specialties, the partnership will also have a significant impact on hospital profitability, both today and in future risk-based payment models.”

Moreover, the PatientBond communication platform provides automated tracking and validation of patient behavior, freeing coordinators to focus on exceptions and non-compliant patients. For instance, coordinators receive notifications if patients are not following protocols or if they skip an appointment. Such exception management is critical for boosting the productivity of navigators as it removes the need to manually validate the compliance of all patients, a time-consuming and relatively low-value task.

Winzenread summed up the partnership: “PatientBond and Cordata share a passion for open collaboration and improving healthcare. Further, both companies embrace the ‘human factor’ in healthcare. Automating communications makes care coordinators more productive and personalizing communications for patients makes them effective. This is the way to drive both better clinical and business outcomes in healthcare.”
